How to Apply
- Download the official application form (Annexure-A) along with Sports Certificate Formats (Form 1 to Form 5).
- Fill in all details in English capital letters with black ballpoint ink, and paste a recent self-attested passport-size colour photograph.
- If applying for both posts (Tax Assistant & Havaldar), indicate your clear order of preference on the single application form (only one application per candidate is allowed).
- Attach self-attested photocopies of Class 10th certificate, educational degree/marksheet, sports certificates issued by competent authorities in the prescribed proforma, photo ID proof, and caste/community certificates.
- Enclose all documents in an envelope super-scribed with: “APPLICATION FOR MERITORIOUS SPORTSPERSON IN CGST & CENTRAL EXCISE LUCKNOW ZONE 2026”.
